Not wanting to tell a store manager about my diagnosis

While I was at work yesterday, I mentioned to one of the cashiers that I was concerned about not being able to afford my prescription medications. I don't remember if she knows about my diagnosis, but I think that she might know that I take medication for my cognition. She said that I should talk to the store manager about this problem because there is a fund for associates. My problem with doing this is the fact that he doesn't know about my diagnosis. As long as I am able to do my job, he doesn't need to know. My question is this, how do I possibly get the help that he might be able to give me without giving him information about my diagnosis? This is really hard!
I did talk to the store manager about my financial problems with paying for my prescriptions. I don't remember whether I told him about my diagnosis. In the end, it was a waste of time because he wasn't able to help me.
