Challenges with filling out a paper at school

Today while I was working at school, a teacher asked me to fill out a paper for a student who I work with. It asked questions such as “does the student work well independently”? I had to circle a number that answered always, occasionally, or sometimes. I think that there were at least twenty questions. I looked at the paper and scanned the questions and answered the ones that I was very sure of my answers. Then came the hard part of answering the remaining questions. I really couldn't figure out what number to choose for the rest of the questions because I couldn't remember. I didn't want to tell the teacher that I couldn't do it, so I really have a problem. After a short time, I noticed that there was another paper underneath the one that I was filling out. It was the same paper that I was filling out, but it contained the teacher's answers for the same student. Although I've never done this before, I decided to choose most of the same answers as the teacher. I thought about each question before I chose my answer and if I thought the teacher's answer was correct, that is what I chose. I left a couple of questions blank because I was unsure of the answer. Then I gave the papers back to the teacher. I know that I did the best that I could.
