Having trouble remembering how to do the exercises at my physical therapy appointment

In December 2014, I had left shoulder surgery. I have been going to physical therapy twice a week since January. My shoulder is frozen, so I don't have full mobility. When I started physical therapy, I explained to my physical therapist that I have problems with my short-term memory. There was no need to go into details, but I knew that he would need to know.
The exercises that I've been doing for quite a while, I think most of the time, I can remember how to do them correctly, but recently he added some new exercises. Last week after I went home, I tried to do one of the exercises. I wasn't quite sure how to do it, so I asked my therapist. He showed me how to do it, but after I had done ten repetitions, took a short rest, then tried to do them again, I had totally forgotten what I had just done, moments earlier. I told my physical therapist and he showed me how to do the exercise again. Luckily, my physical therapist is great! He is very patient and understanding. I'm usually really good at communicating any problems that I'm having such as remembering how to do something, or understanding what he wants me to do.
