Not understanding how to apply for medicaid

(Chapter 14 Comprehension Problems in my book)
Earlier today, I went to the Health and Human Services office to apply for Medicaid, so I can get some healthcare. I've never enjoyed filling out paperwork, but now it is a lot more challenging because of the memory impairment. When I got there, I found a packet of papers, probably about six and a stack of numbers. I looked at the papers, but I just couldn't figure out if that was what I needed to fill out. There was way too much text for me to read and then comprehend, so I think that I mostly scanned it. I took a number and sat down and waited to be called. When my number was called, I walked up to the window. I told the woman that I needed to apply for healthcare and she asked if I filled out the papers. I told her that I didn't know what parts I needed to fill out. She had a copy of the forms and only gave me the ones that I needed to complete. She asked me to fill it out and sign and date where she checked it off. I sat over at a table and filled out the forms the best that I could. I guess there was another small section that I missed because when I went back up to the window, she pointed it out to me. So, I finished filling it out, then handed it back to her. She took the papers and started typing on her computer the information from the form. This was taking quite a while and while I was standing there, my knee was really hurting, so I got a chair and sat down in front of her window. When she finished, she told me that I would get a follow-up in the mail.
I didn't even think to ask for help until my number was called, but then I had no problems letting the woman know that I needed help. She was very understanding and helpful and I appreciated that.
In April 2014, I had a really bad fall at the school where I worked. July 2014, I had left knee surgery and in Dec. 2014, I had left shoulder surgery. Unfortunately, these injuries continue to cause me pain, that is pretty severe! I can no longer work and I'm waiting to settle my case with worker's compensation.
